What Would Actually Fix the Affordable Care Act?

Though Democrats reject Republicans' apocalyptic vocabulary — collapse, death spiral, crumbling — they acknowledge that there are significant problems with Obamacare; members of both parties say that the cost to obtain coverage is far too high, and that the economics of the insurance marketplaces aren't working. So, setting political realities aside, what could be done to fix the law?
