To Fix the Exchanges, Focus on Health, Not Income

The ACA Exchanges have numerous provisions focused on making premiums “affordable.” These provisions include not only subsidies based on family income, but also rate regulation (imposed by the states, but with federal supervision and funding), the prohibition of medical underwriting, and limited subsidies for cost-sharing. However, there is no provision of the ACA that seeks to ensure that providing actual health care to beneficiaries will be “affordable” for insurers.
