While we have spent much of the past decade arguing about the Affordable Care Act and how to improve health care coverage, we have paid very little attention in this same time frame to the physical, social and economic structures that produce health. The health care sector by itself can achieve only so much improvement. Without the engagement of other sectors, we are doomed to have limited health indicators, holding back everyone's quality of life.
